Humana Group Medicare Advantage (PPO) Base Plans
State Health Plan Medicare retirees have several options for health plan coverage. One of these options is the Humana Medicare Advantage PPO & Prescription Drug Base Plans, which includes Medicare prescription drug coverage.
Below are resources for members enrolled in the Humana Medicare Advantage PPO & Prescription Drug Base Plans. Please note that you will have TWO ID cards from Humana in 2026. One is to use for Medical services and one is to use for Pharmacy services. It is possible that you will receive duplicative notices throughout the year as well as they are required by the Centers for Medicaid & Medicare Services.

Members indicated as 50% or 100% contributory notes the percentage of the premium for which they are responsible.
IMPORTANT MESSAGE
You cannot be enrolled in multiple Medicare Health Plans (Medicare Part D and/or Medicare Advantage). Should you (or a covered spouse) choose to enroll in an outside Medicare Health Plan, you will be disenrolled from your Humana Group Medicare Advantage Plans and automatically moved to the 70/30 Plan. This could significantly impact your premiums.
